I can see higher HD humanoids as exceptional within reason. For instance, the standard AD&D orc chief is 3HD, but perhaps something like 1 tribe in 10 has a chief of unusual ability with 4HD. And then perhaps once every few generations (so about 30-50 years for orcs ), one of these rises above his peers and gathers multiple tribes around him as a 5HD monster.
